Discount Medical Plans do not meet the Minimum Creditable Coverage requirements under M.G.L. C. 111M and 956 CMR 5.00. They do not qualify as health plans under the Affordable Care Act. The Discount Medical Plans (Dental, Vision, Chiropractic and Holistic Programs) on the page above are administered by Patriot Health Florida, Inc., a discount medical plan organization. They are not health insurance policies and are not available in all areas. They provide discounts to certain health care providers for medical service and do not make payments directly to the providers for medical services. The range of discounts for medical services provided under these plans will vary depending on the type of provider and service received. The member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with Patriot Health Florida, Inc., Located at 2540 Metrocentre Blvd, Suite #5, West Palm Beach, FL 33407 , 800-292-3797.
